Overview
Documentation is the cornerstone of effective patient care as it is used to record a patient's condition, treatment plan, and progress. It can ensure the continuity of care while also ensuring patient safety and enhancing clinical decision-making. However, vague, biased, and inconsistent documentation can lead to ineffective patient care and reduced outcomes. In this course, you will capture and accurately document important clinical information based on various common scenarios in the context of mental health care. You will analyze the use of language and reframing, the importance of objective documentation, and what information to capture during assessments across disciplines and in various situations.
